Medicare
Individuals with a chronic health condition or complex needs being managed by a General Practitioner are able to claim a rebate for Exercise Physiology services. (Referral from GP required including Medicare referral form). Click here for Medicare referral form.
NDIS (plan and self managed)
MTMEP are able to provide Exercise Physiologist services for NDIS participants who are plan-managed or self-managed. Please complete and return the new referral form to MTMEP and we will be contact ASAP. NDIS referral form. NDIS referral form.
Department of Veterans Affairs
Veteran Gold and White Card - access up to 12 sessions per Allied Health Services treatment cycle (Referral from GP required).
Private Health insurance
In order for a patient to receive a rebate from their Private Health Insurer, the patient’s cover must include exercise physiology services and the treatment must aim to prevent, delay or ameliorate a chronic disease or injury.
